Infertility

Infertility refers to a state where a couple is unable to conceive after attempting pregnancy regularly for twelve cycles without success. It can also be referred to as a biological inability of an individual to contribute to conception or the inability in a female to carry a pregnancy to full term. There are various factors which lead to infertility.

Research indicates that about 20% infertility cases are due to a problem in the male and about 40-50% cases are due to a result of female conditions while the rest are either due to sperm disorders or unidentified factors.

Treatment options for infertility depends on the age of the patients, personal preferences and for how long they have been infertile. However, modern sciences have come up with revolutionary IVF techniques which would fulfill your desire of having a child. Infertility is very painful. Patients with low sperm count , azospermia, low motility or other dysfunctions can choose from various sperm donor profiles and solve their part of infertility. Females with anovulation, unhealthy eggs or any genetic deficiency can opt for ovum donation, embryo transfer and ovum pick up. If the uterus is incapable of sustaining a pregnancy or has fibroids or tumours etc, the couple can opt for surrogacy. We do not charge for the surrogacy. In Vitro Fertilization (IVF)

In vitro fertilization is a process by which the egg is extracted from a woman's ovaries and is fertilized by a sperm in a fluid medium outside the body. The process then involves the implantation of the fertilized egg back into the uterus of the woman for a successful pregnancy.

Procedure

Theoretically, the samples of sperm and egg are collected from the parents, fertilized in the laboratory and the resulting embryos are then replaced in the uterus. However, without additional techniques, the chances of pregnancy would be extremely small.

Ovarian hyper stimulation

There are two current main protocols for stimulating the ovaries for IVF treatment. The long protocol and the short protocol.

Natural and Mild IVF

IVF can be performed by collecting a naturally selected egg from woman's natural menstrual cycle without the use of any drugs. It is known as natural cycle IVF.

The next step-up method is called modified natural cycle IVF. In this method, medication is used for 2-4 days during woman's natural cycle to avoid spontaneous ovulation and to make the treatment more successful.

Mild IVF is a method where a small dose of ovarian stimulating drugs are used for a short duration during a woman's natural cycle aimed at producing 2-7 eggs and creating healthy embryos.

Retrieval and final maturation of the egg

When the ovarian follicles have reached a certain degree of development, induction of final oocyte maturation is performed.

The eggs are retrieved from the patient using a transvaginal technique called transvaginal oocyte retrieval, involving an ultrasound-guided needle piercing the vaginal wall to reach the ovaries.

Preparation of the egg and sperm

In the laboratory, the identified eggs are stripped of surrounding cells and prepared for fertilization. In the meantime, semen is prepared for fertilization by removing inactive cells and seminal fluid in a process called sperm washing.

Fertilization of the egg

The sperm and the egg are incubated together at a ratio of about 75,000:1 in the culture media for about 18 hours. After that the fertilized egg is passed to a special growth medium and left for about 48 hours until the egg consists of six to eight cells.

Embryo culture

Typically, embryos are cultured until having reached the 6-8 cell stage three days after retrieval.

Selection of Embryo

Laboratories have developed grading methods to judge oocyte and embryo quality. In order to optimize pregnancy rates, there is significant evidence that a morphological scoring system is the best strategy for the selection of embryos.

Transfer of the Embryo

Embryos are failed by the embryologist based on the amount of cells, evenness of growth and degree of fragmentation. The number to be transferred depends on the number available, the age of the woman and other health and diagnostic factors.
